The Center on Substance Use and Health (CSUH) at the San Francisco Department of Public Health (“us”, “we”, or “our”) operates csuh.sf.org (the “Site”). This Privacy Policy outlines the types of information we gather when you visit our website, as well as some of the steps we take to safeguard it. 

Information Collection 
We do not collect personal information about you when you visit our website unless you choose to provide that information to us. 
If you choose to share information with us via the Contact Form, we will ask for your first and last name, email address, and phone number. This information will be emailed to us and save on our secure WIX server. If requesting to join a CUSH study, this information will then be transferred to REDCap, a secure data capture software. You can ask to review, change, edit or update the information submitted to us by contacting us at csuh.studies@sfdph.org.

 

Data Collection 

When you visit our site, we may collect aggregated usage Personal Information, such as browsing and ‘click-stream’ activity, session heatmaps and scrolls, non-identifying Personal Information regarding your device, operating system, internet browser, screen resolution, language and keyboard settings, internet service provider, referring/exit pages, date/time stamps, etc. 

 
We use the aggregated information from all of our visitors to measure server performance, analyze user traffic patterns and improve the content of our Site. In addition, WIX does collect essential Cookies in order to operate the website. You may decline all unnecessary Cookie collection at any time. Information Sharing 
If you choose to share information with us via this website, we will not give, share, sell, rent or transfer any personally identifiable information to a third party. 

Do Not Track
